This study aimed to determine the relationship between hemoglobin levels and fatigue levels in chronic kidney disease (CKD) patients undergoing hemodialysis at Gatoel Hospital Mojokerto. An analytical observational design with a cross-sectional approach was employed. The population comprised 210 patients, with a sample of 68 respondents selected using consecutive sampling. Data were collected from medical records and the FACIT-Fatigue questionnaire, then analyzed using the Spearman rank test. The results revealed a significant relationship between hemoglobin levels and fatigue levels (p = 0.000 < 0.05) with a strong positive correlation (r = 0.645). A lower hemoglobin level corresponds to a lower FACIT score, indicating more severe fatigue. Hemoglobin reduction triggers tissue hypoxia, disrupting energy metabolism and leading to severe fatigue. In conclusion, hemoglobin levels strongly correlate with fatigue severity in CKD patients. Hospitals should regularly monitor hemoglobin levels and provide anemia management education to minimize prolonged fatigue
